How Soon Does Ketosis Begin? Diet Ketosis vs a Ketone Spike
Quick answer
Usually two to four days on about 20 to 50 grams of carbohydrate a day. A gram dose of BHB salts is different: blood ketones peak in about an hour and are back to baseline in 3 to 4 hours, and the liver does not start making ketones. SGLT2 inhibitor medicines make low-carb eating riskier, so ask your doctor first.

"How long to get into ketosis" is one of the most searched keto questions, and the answer depends on what you mean by ketosis. Your body can make ketones itself on a low-carb diet, or you can swallow them. The two run on very different clocks, and only one of them is what a keto diet means.
How soon does ketosis begin?
Usually two to four days, if you keep carbohydrate to about 20 to 50 grams a day, says Cleveland Clinic (Cleveland Clinic, Ketosis: Definition, Benefits and Side Effects). That is ketosis made by your own body from a change in diet.
A gram dose of ketone salts works on another clock. Blood ketones rise within the hour and fall back within a few hours (Frontiers in Nutrition, Cuenoud 2020, D-BHB salts pharmacokinetics). The numbers below show both.

What happens in the first weeks of keto?
The body shifts fuel over a few days, and some people feel rough for a week or two while it does. This track joins what Cleveland Clinic, a 2025 review of 89 studies and a 6-week diet trial report. Your own days may run faster or slower.
- Day 1
Carbs drop
Daily carbohydrate falls to roughly 20 to 50 grams. This is the one step you control directly.
- Days 2 to 4
Ketosis begins
For most people this is when blood BHB climbs past 0.5 mmol/L.
- Days 4 to 11
The keto flu window
Headache, tiredness, brain fog, cramps or nausea can start two to seven days after ketosis begins.
- Week 2
Ketones hold steady
In a controlled keto diet trial, fasting blood ketones stayed above 1.0 mmol/L from week 2.
- Weeks 2 to 3
Most symptoms fade
Keto flu usually lasts one to two weeks, then eases.
Sources: the keto flu timing is from Cleveland Clinic (Cleveland Clinic, Keto Flu: What It Is and How To Manage It), the ketosis line from the 2025 review (Frontiers in Nutrition, Skartun 2025, Symptoms when starting a ketogenic diet), and the week-2 levels from the diet trial (Frontiers in Nutrition, Buga 2021, Ketone salts and body composition). The same review notes that early weight loss on keto is mainly water, so a quick drop on the scale in week one is not the same as fat loss.
Is a ketone spike from a pill the same as ketosis?
No. Diet ketosis means your liver is making ketones from fat, day after day. A ketone salt adds BHB to your blood from outside, and it is gone within hours.
| Point | Ketosis from a keto diet | Ketones from a gram dose of BHB salts |
|---|---|---|
| Where the ketones come from | Your liver, from fat | The drink or capsule; the liver does not start making ketones because of it |
| How fast it starts | Two to four days | Within 15 minutes to an hour |
| How long it lasts | As long as carbs stay low | Back to baseline in 3 to 4 hours (D-BHB) |
| Levels seen in studies | Fasting ketones above 1.0 mmol/L from week 2 | Peaks of about 0.6 to 1.2 mmol/L |
| What it takes | About 20 to 50 g of carbs a day or fewer | 3.75 to 24 g of BHB salt per serving in studies |
| Weight result | About 1 to 2 kg more loss than other diets | No added loss on top of a keto diet |
The liver point comes from a 2025 review (Nutrition Reviews, Biesiekierska 2025, Ketogenic nutrition and obesity); the weight results from a Cochrane summary (Cochrane (Naude 2022), Low-carb vs balanced-carb diets, plain summary) and the 6-week salt trial. In that trial, people on the same keto diet reached the same steady ketones with or without the salts.
Does taking ketones put you in ketosis?
For a short time, a gram dose can push blood BHB over the line researchers use for ketosis. After one 3.75 g serving of a sodium, calcium and magnesium BHB blend, blood ketones read 0.7 mmol/L at 15 minutes and 0.4 mmol/L at 60 minutes (Nutrients, Stefan 2021, Sodium/calcium/magnesium BHB salt in adolescents). That study enrolled 10 to 17 year olds and was funded by a ketone company.
A larger single dose of a sodium and calcium DL salt, 0.5 g per kg of body weight, rose slowly to 0.598 mmol/L at 2.5 hours (J Nutr Metab, Fischer 2018, Sodium and calcium BHB salt in adults). One of the six adults dropped out with stomach symptoms.
So yes, briefly, at grams. But a reading over 0.5 mmol/L that fades within hours is not the steady state a keto diet brings. The ketones were swallowed, not made, and the level fell back within hours.
Can medication or alcohol affect ketosis?
Some medicines change how safe ketosis is, and that matters more than whether they "kick you out" of it. None of the sources we cite says a medicine ends ketosis, so we do not claim one does.
SGLT2 inhibitor diabetes medicines
The FDA warns that SGLT2 inhibitors can cause ketoacidosis, even when blood sugar is below 250 mg/dL. In the 73 cases it reviewed, possible risk factors included a low carbohydrate diet or a general drop in calories (FDA Drug Safety Communication, SGLT2 inhibitors and ketoacidosis).
Blood pressure and diabetes medicines
Cleveland Clinic says some blood pressure and diabetes medicines can make keto flu symptoms worse, and advises checking with a primary care provider before you start. Mayo Clinic names diabetes, heart disease and kidney disease as reasons to talk to a health professional first (Mayo Clinic, Low-carb diet: Can it help you lose weight?).
Alcohol
None of the health sources we cite covers alcohol and ketosis, so we give no rule here. Ask your doctor or a dietitian, especially if you take any of the medicines above.
Where does Retride fit in the ketosis timeline?
The Retride label says to take two capsules daily with an 8 oz glass of water, and adds: "For best results in achieving ketosis, aim for a diet ratio of 70% fat, 25% protein, and 5% carbohydrates." Read plainly, the label is pointing at the diet.
The two-to-four-day clock above belongs to that diet. No timeline is printed for the capsules, and no study has measured what a 60 mg blend of BHB salts does to blood ketones. We do not claim Retride raises ketones or causes ketosis.
The diet itself is explained in our keto diet guide. Daily directions are on the how to use Retride page, and what to expect over the first weeks is on the results timeline page.
How long to get into ketosis: FAQ
How soon does ketosis begin?
Usually two to four days on about 20 to 50 grams of carbohydrate a day, according to Cleveland Clinic. Some people take a little more or less time.
How fast do exogenous ketones work?
At gram doses, blood BHB rose within 15 minutes to an hour and was back to baseline in 3 to 4 hours. That is a short rise, not diet ketosis.
Does taking ketones put you in ketosis?
Only briefly, and only at grams. A 3.75 gram salt serving read 0.7 mmol/L at 15 minutes and 0.4 mmol/L at an hour, and the liver does not start making ketones because of it.
What BHB level counts as ketosis?
Researchers call it nutritional ketosis when blood BHB is above 0.5 mmol/L. In a keto diet trial, fasting levels stayed above 1.0 mmol/L from week 2.
Can medication kick you out of ketosis?
Our sources do not say any medicine ends ketosis. They do warn that SGLT2 inhibitors can cause ketoacidosis with a low carbohydrate diet, and that some blood pressure and diabetes medicines can worsen keto flu.
Can you drink alcohol on keto?
None of the health sources we cite covers alcohol and ketosis, so we cannot give a rule. Ask your doctor or a dietitian.
How long does keto flu last?
Usually one to two weeks. It can start two to seven days after you reach ketosis, says Cleveland Clinic.
Is ketosis the same as ketoacidosis?
No. Ketosis is the body running on fat during a low-carb diet. Ketoacidosis is a dangerous build-up of ketones, most common in type 1 diabetes, and it can be life-threatening.
Can Retride put you in ketosis?
No study has tested it. Retride's blend is 60 mg, and the label's own route to ketosis is a 70/25/5 keto diet.
