Do Keto Pills Work? What the Trials and Regulators Say
Quick answer
Not for weight loss, on the evidence we found. BHB salt keto pills raise blood ketones for 3 to 4 hours at gram doses, but about 24 grams a day added nothing to a keto diet's weight loss, and an 8-week trial did not beat placebo. The FTC calls weight loss without diet or exercise a false claim.

"Do keto pills work?" is the question people type most often about keto supplements, ahead of any brand name. The honest answer is less exciting than the ads, and more useful. This post walks through what the pills contain, what trials found, which promises regulators call false, and who should be careful.
Do keto pills work? The short answer
Not for weight loss, on the evidence we could find. At gram doses, BHB salts raise blood ketones for a few hours, but they did not add weight loss to a keto diet (Frontiers in Nutrition, Buga 2021, Ketone salts and body composition), and an 8-week trial with a low-calorie diet did not beat placebo (Nutrients, Roeth 2025, BHB salts with a low-calorie diet).
The NIH Office of Dietary Supplements says evidence is limited for most weight loss ingredients, and that healthy eating, fewer calories and physical activity remain the basis of weight loss (NIH Office of Dietary Supplements, Dietary Supplements for Weight Loss). Keto pills have not changed that.

What do keto pills actually do in the body?
Most keto pills contain BHB salts: beta-hydroxybutyrate, the main ketone your body makes on keto, bound to sodium, calcium, magnesium or potassium. Swallow enough of it and blood BHB rises for a while.
In studies, blood ketones peaked about an hour after a gram-level dose and were back to baseline within 3 to 4 hours (Frontiers in Physiology, Stubbs 2017, Metabolism of exogenous ketones). A 2025 review adds a key point: exogenous ketones raise blood BHB but do not make the liver produce ketones (Nutrition Reviews, Biesiekierska 2025, Ketogenic nutrition and obesity). The ketones come from the pill, briefly, and the body's own fat-burning state is not switched on by them.

Do exogenous ketones work for weight loss?
"Exogenous" means from outside the body. The trials that tested exogenous ketones for weight or eating found little or nothing beyond the diet people were already on.
| Study | What people took | What happened |
|---|---|---|
| (Frontiers in Nutrition, Buga 2021, Ketone salts and body composition) | About 24 g a day of BHB salts on a keto diet, 6 weeks | 7.3 kg lost vs 8.0 kg on placebo |
| (Nutrients, Roeth 2025, BHB salts with a low-calorie diet) | About 10 g a day of BHB salts on a low-calorie diet, 8 weeks; funded by Ketone Labs | 3.05 vs 1.51 kg; not significant (p = 0.09) |
| (Nutrition Reviews, Biesiekierska 2025, Ketogenic nutrition and obesity) | Ketone drinks for 28 days, healthy adults (a study in the review) | Body weight and body make-up unchanged |
| (Int J Exercise Science, Nepocatych 2022, Ketone salt and energy intake in women) | 0.25 g per kg a day of BHB salts, 10 days, 14 women | 1,263 vs 1,335 calories a day; no significant difference |
| (Frontiers in Nutrition, Okada 2021, Ketones and later food intake) | Ketone ester around exercise, 13 adults | Lower ghrelin, but the next meal was about the same (964 vs 997 calories) |
| (Nutrients, Mohib 2025, Exogenous ketosis in adults with disease (systematic review)) | 51 studies, mostly esters and MCTs | Benefits mostly on lab markers; no weight-loss results reported |
The review authors' own summary is plain: exogenous ketones may not directly cause weight loss, and ketone supplements are not a cure for obesity. That is the state of the research, at doses far larger than most capsules hold.
Do keto pills work without diet or exercise?
No. The FTC lists weight loss with no diet or exercise among seven claims that are false, and it states that to lose weight you have to burn more calories than you take in (FTC, Gut Check: A Reference Guide for Media on Spotting False Weight Loss Claims).
The FTC also calls "lose weight just by taking a pill" a scam claim (FTC Consumer Advice, New year, new weight loss scams), and NIDDK lists weight loss with no diet or exercise as a warning sign of a fad program (NIDDK, Choosing a Safe and Successful Weight-loss Program). The same logic covers keto gummies and drinks: whatever the format, a promise of weight loss without changing what you eat is the claim to walk away from.
Are keto pills safe, including for people with diabetes?
For healthy adults, the BHB salts studied mostly caused mild stomach problems at gram doses. The bigger risks come from the keto diet the pills are sold with, and from products that are not what they claim.
| Concern | What the source says | Source |
|---|---|---|
| Stomach upset | At gram doses, 1 of 6 adults dropped out of one study; a high salt dose caused more symptoms than a low one | (J Nutr Metab, Fischer 2018, Sodium and calcium BHB salt in adults) (Int J Sport Nutr Exerc Metab, Stubbs 2019, GI effects of ketone drinks) |
| Type 1 diabetes | Diabetic ketoacidosis is most common in type 1 and can be life-threatening | (CDC, Diabetic Ketoacidosis) |
| SGLT2 inhibitor medicines | Ketoacidosis can occur even with blood sugar below 250 mg/dL; a low carbohydrate diet was a possible risk factor | (FDA Drug Safety Communication, SGLT2 inhibitors and ketoacidosis) |
| Heart disease | Mayo Clinic says check with a health professional first; keto raised LDL more than low-fat in a meta-analysis | (Mayo Clinic, Low-carb diet: Can it help you lose weight?) (Br J Nutr, Bueno 2013, Very-low-carb keto diet vs low-fat diet (meta-analysis)) |
| Hidden ingredients | Many diet pills are likely contaminated, and the FDA's list covers only a small fraction | (FDA, Weight Loss Product Notifications (tainted weight loss products)) |
If any row applies to you, talk to your doctor before you start, and bring the bottle. These are general points about keto pills and keto diets, not findings about one brand.
Retride's whole blend is 60 mg, far below the gram doses that upset stomachs in studies. Its label asks anyone pregnant or nursing, or with a known medical condition, to consult a physician first, and it is not for anyone under 18. Every side effect we found is on the Retride side effects page.
Where does Retride fit in this evidence?
Retride is a keto pill of the BHB salt type, with a 60 mg blend of magnesium, calcium and sodium BHB per serving. That is tens to hundreds of times less than the doses studied, and no study has tested the product itself.
Its label suggests a 70% fat, 25% protein, 5% carbohydrate diet "for best results in achieving ketosis." Any ketosis on that plan comes from the food. Retride is a dietary supplement, not a drug, not retatrutide, and not FDA approved.

Do keto pills work? FAQ
Do keto pills work for weight loss?
Not on the evidence we found. BHB salts at about 24 grams a day added no weight loss to a keto diet, and an 8-week trial with a low-calorie diet did not show a significant difference from placebo.
Do keto pills really work, or is it the diet?
It is the diet. Ketosis comes from cutting carbs, and in a 6-week trial the placebo group on the same keto diet lost as much weight as the group taking ketone salts.
Do exogenous ketones work?
They raise blood BHB for a few hours at gram doses. They have not been shown to cause weight loss, and a 2025 review says they may not directly cause it.
Are exogenous ketones worth it?
For weight loss, the trials give little reason to pay for them. Their measured effect is a short rise in blood ketones, not added fat loss.
Do keto pills work without exercise?
No pill replaces eating fewer calories than you burn. The FTC calls weight loss without diet or exercise a false claim.
Do keto gummies work without a keto diet?
The research on BHB does not support it, whatever the format. Weight loss without changing what you eat is a claim the FTC lists as false.
Are keto pills safe for people with diabetes?
Talk to your doctor first. A keto diet raises ketoacidosis risk with type 1 diabetes and with SGLT2 inhibitor medicines, and the FDA names a low carbohydrate diet as a possible risk factor.
Are keto pills safe for heart patients?
Ask your doctor first. Mayo Clinic names heart disease as a reason to check before a low-carb diet, and a meta-analysis found LDL cholesterol rose more on keto than on low-fat diets.
How do keto pills work?
Most contain BHB salts, a ketone bound to a mineral. At gram doses blood BHB rises for 3 to 4 hours, but the liver does not start making its own ketones because of it.
